You will have regular visits, either by telehealth (video or telephone) or in person with the\u00a0<em>nurse navigator<\/em>, which are in addition to the other study visits.\u00a0<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section (AKI)?<\/h3>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-df7c398 elementor-widget elementor-widget-text-editor\" data-id=\"df7c398\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p><em>\u2022\u00a0 AKI is the sudden worsening of kidney function over<\/em> <em>hours to days and is a common problem in patients<\/em> <em>who are in the hospital.<\/em><\/p><p><em>\u2022\u00a0 AKI causes a build up of waste products in your<\/em> <em>blood and makes it hard for your kidneys to keep the<\/em> <em>right balance of fluid in your body.<\/em><\/p><p><em>\u2022\u00a0 After having AKI, your chances are higher for other<\/em> <em>serious health problems (such as kidney disease and<\/em> <em>heart failure), rehospitalization or having AKI again in<\/em> <em>the future.<\/em><\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"elementor-column elementor-col-16 elementor-top-column elementor-element serious kidney<\/em> <em>problems that may lead to dialysis or the need for a<\/em> <em>kidney transplant.<\/em><\/p><p><em>\u2022\u00a0 There is a critical need to study new approaches to<\/em> <em>managing AKI &#8211; if there is a better way to help<\/em> <em>patients manage this condition, we can positively<\/em> <em>impact those who experience hospitalization with acute kidney injury (AKI). Individuals who enroll in the study will be separated into two groups. Group 1 receives a care team approach\u00a0 from\u00a0 a group that includes a nurse navigator, pharmacist, and nephrologist (kidney doctor), plus home blood pressure and weight monitoring devices.\u00a0 Group 2 receives care that is current and typical for AKI (referred to as usual care). These will be done prior to discharge from the hospital, at 90 days, 6 months, and one year.\u00a0 Additionally, questionnaires will be done at 30 days. \u2022 receive information about AKI and recommendations to receive care from the usual providers. \u00a0 The\u00a0nurse navigator\u00a0will be the primary contact for participants in Group 1 and will work to improve communication with the team and your providers.
